INSTRUCTIONS :

1-)Our client …/…/……………………. Manufacture ………………………. the model…. he handed over his antique carpet to the defendant company for washing, and due to this incident, a total of ….. He paid TL service fee, but in return he was given a cash register receipt for the sale of cleaning november instead of a service fee invoice. (APPENDIX – 1)

2-)The carpet subject to the lawsuit has been delivered to the respondent company …. it was delivered to our client on /…/… a week later. (ANNEX – 2) However, our client has determined that the carpet subject to the lawsuit has faded after cleaning and has lost its original state on the date of delivery to the defendant and has applied to the relevant company for the elimination of this damage.

3-) Although the defendant company took subsequent actions to repair the damage on the carpet, the defendant company, which was not successful, left the carpet by leaving the carpet in front of the door with the help of its employees, despite our client’s refusal, and because of the incident, our client and his wife issued two minutes that the company employees refused to sign. (APPENDIX – 3)

4-)For the matter explained above, by us …….. An application has been submitted to the Arbitration Committee for Consumer Problems of the District Governor’s Office, and the Arbitration Committee for Consumer Problems …/… /… date and ….. in accordance with the law No. 6502 with the decision No. 6, it was decided to pay the defendant company’s money at the market value of the carpet. 5-) This decision of the arbitration committee by us to the defendant company was notified to the defendant on the date of /…/… and the carpet, whose m2 is ……TL, has a market value of ………. TL’s ….. it was requested to be paid within the day, but within the recognized period, the defendant did not respond to the company’s warnings, nor was the payment made. (APPENDIX – 5)

5-) In order to cover the material and moral damage suffered by our client to some extent, it has been necessary to open this case.
